Biennial Survey Deadline Approaching

November 30, 2009 is the deadline for the Biennial Survey to be completed and submitted by ALL Federal depository libraries by. It is through this mechanism that depositories fulfill their legal obligation to "report to the Superintendent of Documents at least every two years concerning their condition" (44 USC 1909).

The survey is online. To login:

User: Depository library # (e.g., 0064D)
Password: GPO-assigned internal password

Instructions

  1. Login;
  2. Print the survey questionnaire;
  3. Answer the questions on the printed survey;
  4. Review the answers with your director;
  5. Login to the survey and transfer the answers from the printed survey to the online survey; and
  6. Retain your printed survey responses for your records. Print the last page of the survey that states, "Those are all of our questions" for verification that your survey was submitted.

Tips

  • Once you log in to the survey you can go forward and backward in the survey by using your browser’s “back” button.
  • You may start the survey and return later, but will only be able to access from where you left off to the end.
  • Questions that ask for numbers are programmed as numeric fields and as such will only accept numbers. Do not use any comma separators, just use numbers.
  • If you need to go back and change answers after submitting the survey, you may do so by “resetting” the survey for you library from the login page.
