Reminder: Submit Form to Access Public Health Reports

The dissemination format for Public Health Reports will be switching from paper to electronic only on January 11, 2010. In order to ensure your depository has electronic access on that date, you must submit to GPO a completed account validation form that is signed by your library’s director by December 18, 2009.
